New Organization Driving Health Equity for Intellectually/Developmentally Disabled (IDD)

Helps Other Organizations Help Their Patients, Clients, and Members
with IDD Succeed in the World on Their Own Terms

Individuals with intellectual and developmental disabilities (IDD) face a number of social and systemic challenges to their health, well-being, and quality of life. In addition to shorter life expectancy and prevalence of chronic conditions, they face stigma, exclusion, and inadequate access to disability-competent services.

Partnership Solutions was founded to support the capacity of other organizations of all types to help individuals with intellectual and developmental disabilities succeed in the world on their own terms and participate – emotionally, socially, and economically – as active members of family and community. 

Founded by a team with deep commitments and operational experience serving the IDD community and those that care for, and about, them, Partnership Solutions offers services, resources, and expertise to help other organizations plan, access, coordinate, and integrate services including medical, habilitation, behavioral/mental health, housing and social supports, community services and employment.

Partnership Solutions currently provides services to two NY-based organizations:

  • Care Design NY (www.caredesignny.org), a nonprofit Care Coordination Organization/Health Home serving 30,000 children and adults with intellectual and developmental disability that assists them and their family/representatives to access services that support healthy, well–rounded and fulfilling lives. 

  • Partners Health Plan (www.phpcares.org), a nonprofit demonstration project authorized by the Centers for Medicare and Medicaid Services (CMS), the NYS Department of Health (NYSDOH) and the NYS Office for People with Developmental Disabilities (NYSOPWDD) to test an IDD-specialized managed care model of disability-competent medical, habilitation, behavioral/mental health, social and community services under a single model of care. 

Partnership Solutions is actively seeking to partner with other organizations that seek to improve their capacity to serve those with IDD and is also developing a portfolio of additional service lines in direct services, network development, education and training, and administrative support for other organizations.  This week marks the launch of a quarterly Clinical Brief newsletter for healthcare and developmental disability professionals that reviews and summarizes important studies and other clinical considerations for disability-competent care.
